Jack W. Cook

Jack W. Cook, longtime resident of Worthington passed away Tuesday December 15, 2020 at Putnam County Hospital in Greencastle.

Born March 16, 1931 in Bloomfield the son of Emery and Clovia May (Flake) Cook.

Jack was the owner and operator of Cook’s Food Center and 67 Inn Motel in Worthington, and was a self-employed farmer and land owner. Jack was a U.S. Army veteran of the Korean War; his unit was attached to the Airforce Scarwaf Division.

Surviving relatives include his children, Jack L. (Janis) Cook of Gulf Breeze, Florida; Jana L. (Greg) McIntosh of Bloomfield, John G. (Pam) Cook of Cloverdale, his grandchildren, Elisha Frock, Jonathan (Jasmine) Cook, Lindsey (Dave) Hanna, Justin Cook, Kasie L. (Jarod) Helms, Clint G. McIntosh, Heather (Curt) Wilhoite, Holly Watkins, Amanda Whelchel, Allison (Mitchel) Moy, Jack has twenty-eight great grandchildren and one brother, Charles Cook.

Jack was preceded in death by his wife JoAnn Cook on May 10, 2020; his siblings, Max, Marvin, Herschel, James, Gleddis, Orley Cook, Ruth Fields, Pat Stewart, Dorothy Sexton and Mary.

Graveside services are being planned.
